We are a home health care nursing agency. I am looking for a Nursing Manager/Administrator to help me with the set-up and the accreditation process from CHAPS to recommend my agency for Medicare approval to CMS, Center for Medicare & Medicaid services. I currently own and operate a non-medical Home Care Agency and working in home care since 2015. I have received my initial approval from Medicare and I am going through the steps of accredidation with CHAPS. My office is in Quincy, at 500 Victory Rd. I am looking for 3-8hr shifts., will consider 40 hrs. Your role will be: To help me with the set-up and the accreditation process from CHAPS to recommend my agency for Medicare approval to CMS, Center for Medicare & Medicaid services. Assist with the planning, coordination, leading, controlling, and evaluation of home health services. Coordination and liaison with appropriate parent and affiliate Departments and committees, development of policies and guidelines for and obtaining input from service personnel, and providing professional input, to related administrative systems. Coordinate orientation and in-service programs. Establish relationship with rehabilatation facilties and social workers. Visit new patients, per doctor's order., on board onto Alora Software Prepare admission paperwork for agency. Discuss medication profile with patient. Maintain weekly visits per doctors orders, and discharge. Follow up on other disiplines. What You Will Do: Participate with the patient, and the physician, in discharge activities. Obtain a medical history, particularly as it relates to the present condition. Analyze patient data to determine patient needs or treatment goals. Build and maintain relationships with clients and key personnel within our client companies. Maintain the patient's re-hospitalization referrals or admissions form sheets, Participate in audits, U.R. interdisciplinary team conferences and represent the Agency, in the community, as requested. Assist with the orientation of new professional staff, to familiarize them with the Agency policies and procedures. Adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations. Maintain an on-going knowledge of current drug therapy Develop, and implement, the nursing care plan. What You Will Bring: Our ideal candidate will have 1-3 years of relevant experience and the following credentials/skills: Bachelor's Degree Must be a Registered Nurse licensed by the state of Massachusetts Must have a minimum of 2 years nursing experience, one of which is supervisory, preferably in community healthcare. Must have current CPR certification. Online certification is not accepted Nursing services, treatments, and preventative procedures, requiring substantial specialized skill and ordered, by the physician. The initiation of preventative and rehabilitative nursing procedures, as appropriate, for the patient's care and safety.
