Summary
Works collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care program for all consumers. Is an integral member of the PACT team who works to ensure safety, best practice and high quality standards of care are maintained across the program. Coordinates a wide range of self-management support and disease registry activities for the entire consumer population. Conducts physical health assessments and maintains consumer medication regimen according to Psychiatrist orders.
Essential Functions and Responsibilities
- Demonstrates ability to recognize and assess emergency needs of consumers and initiates appropriate emergency procedures as needed
- Demonstrates current level of knowledge of various payor regulations by functioning within those guidelines
- Acts to facilitate interdisciplinary communication. Utilizes other members of the team in assessing the consumer and analyzes consumer data. Assesses learning needs of the consumer and significant others. Gives accurate and complete information to consumer and family
- Evaluates consumer and family understanding of treatment plan and/or instructions
- Maintains appropriate technique for preparation, procedures and medications administered
- Accurately and legibly documents all consumer interactions in consumer record according to agency procedure
- Reports results and pertinent information to consumer and team members
- Applies professional nursing skills in the provision of preventive health maintenance and treatment of ill and injured consumers
- Conducts comprehensive assessment of consumer’s physical health history
- Participates in daily staff organizational meetings and treatment planning review meetings
- Provides on-call services and serves as backup to evening and weekend staff
- Ensures PACT team has constant nurse coverage
- Provides nursing trainings to OKC PACT team and Shawnee PACT team
- Takes the lead role or participates in providing medication administration and medical services, including administering, evaluating and recording psychotropic medications prescribed by the team psychiatrist; evaluates and charts psychotropic medication protocol
- Develops, revises, maintains and supervises team psychopharmacologic and medical treatment and medication policies and procedures
- Under the direction of the team psychiatrist:
- Manages pharmaceuticals and medical supplies
- Organizes and manages they system of getting medication to clients and integrating medication administration tightly into clients’ individual treatment plans
- Coordinates, schedules and administers medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment
- Provides direct clinical services to clients on an individual, group, and/or family basis in the office and/or in community settings to teach behavioral symptom-management techniques to alleviate and manage symptoms not reduced by medication and to promote person growth and development by assisting clients to adapt to and cope with internal and external stresses
- Attends staff meetings, workshops, and seminars to learn agency policy, rules, regulations, and procedures; participates in on-going in-service training as well as required external training
- Maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment
- Assists in the development and implementation of programmatic goals and objectives
- Supports and assists with the facilitation of Care Plans for consumers within the Red Rock Health Team
- Performs other duties as assigned
Qualifications
- Registered Nurse in the state of Oklahoma
- At least one year of mental health experience or work a total of 40 hours at a psychiatric medication clinic within the first three months of employment
- Valid Oklahoma driver’s license, clean driving record, reliable transportation
- Excellent verbal and written communication skills
- General skill in exercising independent judgment, tact, patience, and understanding; maintaining effective working relationships with clients, other staff, and the community
- Commitment to the mission of Red Rock BHS
- Two years relevant experience preferred
