Responsibilities
Individuals within this job classification are accountable for assessing, planning, implementing, evaluating, and documenting patient related care. They will work with a multi-disciplinary team to provide patient centered care. Will assist in the delivery of timely preventive care, care coordination, and management across departments and vendors. Will collaborate with direct care nurses, hospitalists, case managers, patients, caregivers, PCP providers, and Provider’s Care Coordinators. Will perform other duties as needed.
Qualifications
Job Requirements:
- Graduate of an accredited nursing school
- Current OK LPN License Required
- BLS
