Responsibilities:
- A ctively participates in nursing department meetings, town meetings, employee opinion survey, provide communication and feedback with development, implementation and evaluation of delivery of care for senior life communities.
- A ctively participates in self development by attending educational programs and has completed annual hours of continuing education as required for assisted living regulations.
- A ctively participates in the performance evaluation system.
- S erves as mentor to newly hired staff completing evaluations, attending meetings, and demonstrating care.
- C ommunicates creative ideas and methods to other team members for continuity.
- S erves as clinical partner ot the nurse giving input into challenging care issues.
- D emonstrates a positive caring attitude as evidenced by relaxed, pleasant facial expression and gentleness during care of resident.
- S pend free time with residents providing conversation and companionship.
- M aintains a clean and safe work environment for residents and staff. Responsible for equipment assigned for resident care is clean.
- R eports and removes any equipment that is not functioning properly.
- A cts to prevent accidents and incidents.
- P romptly answers pull cords in a timely manner.
- A nticipates resident needs, checks on resident regularly and demonstrates cooperative caring attitude when assisting residents.
- P rovides orientation/reordination of residents to their environment and provides redirection as appropriate for residents level of understanding.
- P rovides resident privacy by knocking on doors and greeting residents prior to entering room.
- D iscuss personal issues with resident, family, co-workers in a private space/area in a professional manner.
- R eports all unusual conditions, occurrences, complaints or reactions to the nurse who has responsibility for that resident in a timely manner.
- R eports and/or documents any changes observed in condition or behavior of resident throughout shift.
- S erves as clinical partner to the nurse giving input into challenging care issues.
- C ommunicates creative ideas and methods to other team members for continuity.
- S peaks to residents in a pleasant helpful (resident center) manner, avoiding talking in halls or common areas with other staff related to resident care needs or other issues such as staffing, assignments or concerns.
- M odels professionalism in communication using sensitivity, active listening techniques, and diplomacy with all customers.
- U nderstands and implements the principals of infection control, consistently using standard precautions and washes hands after each resident contact.
- R emoves and disposes of gloves after each resident contact.
- Second assist as needed for minimal lift transfers
- Assist with activities
- Encourage self-help abilities
- Clean personal hygiene equipment used in the care of the resident
- Assist with ambulation, lifting, moving and transporting of resident, including pushing a wheelchair
- Perform incontinence care
- Assist residents with toileting
- Restock supplies in resident apartments
- Complete resident status checks evey on hour in memory care and evey two hours in AL
- Encourage resident participation in activities offered by the community
- Serves meals and assist with dining service as necessary, supervise dining room during meals to ensure resident safety
- Provide housekeeping services including laundry, making beds, empty garbage, and stocking supplies
