Title: Nurse Practitioner

 

Accountability: Medical Director

 

Qualifications:

Must be a graduate of an accredited school of Nursing and registered with the California Board of Registered Nursing. Must have current license to practice as a Nurse Practitioner, including BRN furnishing number and DEA license. Experience working with behavioral health populations preferred, including persons with SPMI. Must be experienced working with low income, diverse population. Must be experienced with the care and management of chronic health conditions.

 

Skills and Abilities:

Must demonstrate an understanding of the interrelationship between physical medical and psychiatric concerns. Must be skilled in working as a team and possess strong communication and advocacy skills. Must be a champion of healthy lifestyles and preventative care. Must be organized, dedicated, and innovative. Must demonstrate compassionate professionalism with a focus on patient centered care. Must demonstrate flexibility, adaptability, and creativity.

Bilingual Spanish speaking preferred.

 

Duties and Responsibilities:

· To provide quality care for population base of persons with SPMI in accordance with California state practice guidelines.

· To work cooperatively and collaboratively with SCHARP CCBHC Team.

· To work closely and collaboratively with SCHARP behavioral health staff in meeting the specific needs of the SPMI population, particularly regarding the care and management of chronic health conditions.

· To be responsive to patient needs and provide care that is accessible and flexible.

· To serve as an advocate for overall health, prevention, and wellness.

· To work collaboratively with staff in conduction and/or evaluating medical screening.

· To order and evaluate test results, diagnose patients, prescribe medication, and provide medical input into Integrated Care Planning.

· To serve as a consultant with staff on issues related to overall health and wellness

· Liaison, as needed, with other on-site physicians/psychiatrists, community physicians, specialty care providers, pharmacies, insurance companies and drug companies.

· Provide services with a strong recovery orientation, cultural sensitivity, and awareness of the unique needs of the SPMI population.

· Work within a system of fully electronic record keeping and medical records assuring a comprehensive health history and up to date health and medication information is recorded in the consumer’s electronic health record.

· To attend ongoing training as appropriate.

· To perform other duties and responsibilities as assigned.

Southern California Health & Rehabilitation Program (SCHARP) was founded February 1994 as a non-profit entity for the purpose of providing mental health and rehabilitation services to vulnerable ethnic minority populations in Southern California Los Angeles Founded by two African American psychiatrists who were disillusioned with the traditional services provided to severely and persistently mentally ill individuals who are members of minority groups, they targeted this population, along with homeless mentally ill and homeless dually diagnosed. The mission of Southern California Health and Rehabilitation Program (SCHARP) is to provide quality mental health and social services to vulnerable populations in South Los Angeles.
