OPEN POSITION: 
- Nurse Practitioner, Geriatric Primary Care
- Option for House Calls + Facility Visits
- Option for SNF + LTC Visits (no home visits)


SCHEDULE: 
- Full-Time
- Monday through Friday
- No Nights or Weekends
- Some Flexibility Offered


COMPENSATION:
- $100,000 to $115,000 Base Salary, dependent upon experience
- Quarterly Bonuses ($20k annually)
- $120,000 to $135,000 Total Income

LOCATION: 
Terre Haute, Indiana


COMPANY PROFILE: 
Geriatric primary care practice serving assisted and independent living facilities (ALFs/ILFs), as well as private homes.  The practice was founded and is led by a physician with 20+ years of experience in the home-based primary care field.  They currently employ a team of more than 20 physicians and nurse practitioners, who are supported by dedicated clinical and administrative team members.  They are hiring due to growth in their Terre Haute market.


POSITION DESCRIPTION: 
Serve as the primary care provider for a dedicated geriatric patient panel of 200 to 225 patients.

The practice believes strongly in proactive medicine, with a clinical focus on:

- preventative care
- chronic disease management
- medication management
- routine geriatric primary care

Providers typically see patients once a month, but this is adjusted per provider request based on patient needs.

Providers typically travel to 2 buildings each day and see 10 to 11 patients, for a total of 50 to 55 visits per week.  The practice has positions available that are a mix of house calls and facility visits, as well as positions that are solely based in facilities (SNF / LTC with no home visits).

The practice uses the GEHRIMED electronic health record (a mobile-based platform specialized to this type of setting).

Providers share on-call responsibilities.  The current rotation for full-time providers is 1 week every 4-5 months.  On-call is phone triage only and volume is light.

REQUIREMENTS:
- Indiana NP license
- DEA
- 1+ year of advanced practice experience
- previous experience in home care, geriatrics, or a relevant area
