Family Nurse Practitioner

Job Overview:
We are seeking a skilled and compassionate Family Nurse Practitioner to join our healthcare team. As a Family Nurse Practitioner, you will provide comprehensive primary care services to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. This is a part-time position.

Duties:
- Conduct thorough patient assessments, including medical history, physical examination, and diagnostic tests
- Develop and implement individualized care plans for patients based on their specific needs
- Diagnose and treat common acute and chronic conditions
- Prescribe medications and treatments within the scope of practice
- Educate patients on preventive care measures, healthy lifestyle choices, and self-management techniques
- Collaborate with other healthcare professionals to coordinate patient care
- Provide counseling and support to patients and their families
- Document patient encounters accurately and timely in electronic medical records (EMR) system
- Utilize utilization management techniques to optimize healthcare resources

Experience:
To be successful in this role, you should have the following qualifications:
- Valid certification as a Family Nurse Practitioner
- Experience working in a medical office or similar healthcare setting
- Familiarity with Durable Medical Equipment (DME) utilization and management
- Experience working with individuals with developmental disabilities is a plus
