COMMUNITY HEALTH CARE SYSTEMS, INC.
NURSE PRACTITIONER JOB DESCRIPTION
JOB SUMMARY: Provides primary healthcare and performs selective medical services under the
direction of center physician.
JOB CLASSIFICATION: Exempt.
RATING DUTIES AND RESPONSIBILITIES:
A. Provides primary healthcare and performs selective medical services.
Assess the health status of individuals and families through history taking, physical examination,
and definition of health and development problems.
Make interventions to maintain, restore, or improve the health of individuals and families by the
use of protocols, that is, as authorized by the Medical Director.
Record findings and management plan in systematic form to facilitate professional use of data in
providing quality care and in evaluating care through audit.
Promote independent positive health behaviors by involving the individual in their plan of care.
Provide continuity of healthcare by working in collaboration with the Medical Director and other
health professionals in rendering comprehensive services through the development of a referral
mechanism.
Provide community outreach activities by educational and or screening sessions in the community
as directed by the Medical Director.
Dispense drug samples and medications as directed by the Medical Director.
Order the appropriate labs, x-rays, and diagnostic test. The results will be reviewed by the
supervising provider.
Maintains CME requirements.
Assigns medical records to supervising physician in a timely manner per CHCS, Inc. policy.
Participate in coordination of care for individual patients. (PCMH)
Support patients and families in self-management, self-efficacy, and behavior change. (PCMH)
Fulfill roles and functions of a care team member under Patient Centered Medical Home. (PCMH)
Provides care specific to the education, physical, psychosocial, cultural, and developmental/age
specific needs of the patient. (PCMH)
Observes the communication abilities and needs of the patient and structures procedures,
education, and care accordingly. (PCMH)
Supports follows Family Planning Policy.
Keeps documents, labs, telephone notes and referrals reviewed and handled timely.
Performs other duties as assigned.
B. Manages environment of care in safe and efficient manner:
Ensures work area is kept clean and orderly.
Reports equipment failure or hazardous environment conditions to appropriate person promptly.
Complies with Center policies related to safe environment and employee/patient health and safety.
Responds appropriately to emergencies or drills.
Demonstrates frequent hand washing using good technique.
Practices good body mechanics and ergonomics when performing duties.
Keeps work area stocked with appropriate supplies.
C. Properly maintains accurate and complete medical records of various media forms (including written,
computer generated, audio, video, or other means) as per CHCS policies and HIPAA regulations:
Uses only approved abbreviations.
Documents assessment findings, interventions, and evaluations in medical record in a timely,
accurate and complete fashion.
Maintains integrity of medical record by compliance with policies for medical record entry
corrections and late entries.

Reports actual or potential breaches of confidentiality to appropriate personnel.
Chart Review: Assigns 25% of the medical records quarterly for review.
100% of narcotic records /adverse event for review by the supervising physician.
Timely completion of Medical Record.
D. Demonstrates competency in skills:

Attends and participates in in-services/continuing education activities as directed.
E. Demonstrates professionalism:
Upholds the confidentiality of all patient/provider information.

Follow the Center's dress code.
Demonstrates responsibility for maintaining unquestionable attendance record.

Gives and receives constructive criticism in a professional and respectful manner.
Demonstrates ability to prioritize work and meet deadlines.
Maintains confidentiality of Center business and quality improvement activities.
Participates in improving organizational performance activities as requested.
Regularly attends and participates in staff meetings and work team as assigned.
Participates in orientation of new staff members as requested by supervisors.

MINIMUM QUALIFICATIONS:
Masters degree in nursing with specific training as a Nurse Practitioner and current certification by the
ANA and the Georgia Board of Nursing as a Nurse Practitioner. Current unrestricted license to practice as a
Registered Nurse in Georgia. Three years experience in clinical practice preferred.
TYPICAL PHYSICAL DEMANDS AND WORKING CONDITIONS:
Good general health. Requires prolonged sitting, some bending, stooping, kneeling, crouching and
stretching. Hand-eye coordination and manual dexterity sufficient to operate a keyboard, photocopier,
telephone, and other office equipment of the tasks assigned. Requires adequate eyesight. Occasional
lifting and carrying of up to 15 lbs. Possible exposure to potentially toxic substances: electrical,
flammable, explosive gas hazards; and other conditions common to a clinic environment. Some work areas
are small. Noise may be moderate. Requires working under stressful conditions. Requires exposure to
unpleasant elements common to health care (illness, emotional distress, trauma, etc.). Is able to perform
job functions with proper technique.
