This role involves conducting patient consultations and assessments via Telehealth and collaborating with healthcare providers for ongoing patient care. Your responsibilities include documenting patient progress and maintaining accurate records. To thrive in this role, you should possess a current Nurse Practitioner license in the state of GA and be credentialed with the states Medicaid/Medicare programs, along with excellent communication and collaboration skills.
