Position: Nurse Home Visitor- Post Natal NHVP
Dept: Post Natal NHV Program
Shift: Exempt; FT Days
Primary Purpose:
Nurse Home Visitors provide high quality nursing care to patients in their home. The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate patient care delivery, that focuses on a particular patient population(s)/specific health condition, by the health care team. Using the nursing process, the RN assesses the patient, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. The RN utilizes knowledge of patient needs and the healthcare environment to assist patients to transition through the healthcare encounter without any preventable complications or delays. The RN delegates interventions to health care personnel based on the Texas Nursing Practice Act, each patient's condition, and the competencies of the employee.

Job Summary:
- Identifies ways to improve work processes and improve customer satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals of the department and Parkland.
- Stays abreast of the latest developments, advancements, and trends in the field by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.
- Maintains knowledge of applicable rules, regulations, policies, laws and guidelines that impact the area. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
- Must be able to communicate and collaborate effectively with a diverse group of patients, families and healthcare staff.
- Must be able to demonstrate a working knowledge of specific patient populations, and be able to demonstrate knowledge of disease processes affecting this group.
- Must be able to demonstrate a working knowledge of PC operations and the ability to use word processing software in a Windows environment.
- Must be self-directed and capable of priority setting and problem solving.
- Must be able to demonstrate patient centered/patient valued behaviors.
- Must have reliable vehicle transportation for home visits.
- Must be able to maintain a driving record that meets the criteria established by Parkland procedures.
- Must be familiar with traffic regulations and locations in the surrounding areas.
Minimum Qualifications:
- BSN Degree from an accredited Professional School of Nursing.
- Current/Valid State of TX RN license or valid temporary permit; or valid Compact RN license in good standing without stipulations.
- One (1) to two (2) year's direct registered nurse experience in maternal/child health, public health and/or home visitation.
Certification/Registration/Licensure:
- Current/Valid Basic Life Support (BLS) for Healthcare Providers certification card upon hire and the ability to maintain thereafter from one of the following: American Heart Association, American Red Cross, or Military Training Network.
- Current/Valid State of TX Class C Driver License.
- Current/Valid Automatic Background Check through Texas Department of Family and Protective Services upon hire and required completion every 2 years.
- May be required to have additional specialty certifications.
