Responsibilities :
- Supervising HHA and Companion Staff
- Perform Initial Client Assessments for new clients.
- Establish and maintain Protocols to comply with agency accreditation requirements.
- Develop initial Client Care Plans based on your assessments.
- Evaluate client records relating to the care provided to the client on a regular basis.
- Conducting service calls with the client and/or their responsible party every 30 days ensuring the Plan of Care continues to meet client needs.
- Perform in-person client re-evaluations every 60 days or as warranted by changes in the clients condition or when the client returns home following a hospitalization.
- Conduct and manage competency testing, clinical supervision, and complete performance evaluations for caregivers.
- Perform Caregiver introductions and training on the clients care plan.
- Assist in interviewing and recruitment of Home Health Aides evaluating skills, attitude, and personality of Home Health Aides as needed.
- Maintain good working relationships with clients, family members, and caregivers, focusing on retention (retaining both clients and caregivers).
- Documenting results of Initial and follow-up assessments in agency systems.
- Discharging clients
- Preparing and at times Executing Lunch 'N Learn and/or in-services.
- Conduct orientation and various Skills Training for HHAs and other staff members as required.
- Valid RN License in NJ with minimum experience of 2 years
- Ability to travel to Client location (Central-North NJ) for assessments
- Familiarity with NJ Home Care Agency regulations and accreditation requirements
- Demonstrated ability to work as a team leader.
- Knowledge of Home Care, Geriatrics care is a big plus
- Ability to listen and communicate well with others.
- Computer skills, comfort with technology.
- Some non-traditional work hours are required to meet the needs of individuals.
- Must have at least 2 years of Experience in Home Care/Home Health.
- Criminal Record Clearance.
- Are currently certified
- Hold a valid driver's license, auto insurance, and a clean driving record
- Have a smart phone and email account
- Understand the importance of ethics and client confidentiality
