75 MILE STRAIGHT LINE RADIUS RULE (not driving distance) - NO LOCAL TRAVELERS -Please only submit candidates that area aware of their submission to our facility. Please note time off requests must be disclosed with initial submission to be considered for approval. Time off requested after the acceptance date will not be approved. When submitting candidates include at least two references (one must be from current manager), a resume, and completed skills checklist/competency assessment. ***Please Note - All Requested Time Off (RTO) must be disclosed at the time of submission. If your agency or the traveler does not include all RTO in the submission form and adds it post confirmation, your agency will be at risk of being removed from the PHS vendor set. No exceptions! Time off requested after the acceptance date, will not be approved.13 weeks assignment, 2 years experience current required in a small PACU and have Pre-Op experience as well. Must be a team player, flexible and accustomed to fast paced environment. Must have successfully completed at least one 13-week travel assignment.CERTS: BLS, and ACLS certifications required. BLS and ACLS must be completed through American Heart Association, American Health and Safety, or American Red Cross. EPIC EMR experience preferred.SHIFTS: 8 hour DAY shift, position; shift start 07:00 On call, weekends and holidays are required, 15 days of call per month with 2 weekends included in 15 days, 20 minute call back time enforced. Traveling pairs are not guaranteed same schedules or same shifts.ORIENTATION: 1st week of the contract, not to exceed 40 total hours, and includes EPIC training. Scrubs required and provided by the facility. Please review the facility orientation requirements when submitting for this job. Traveler clearance required 7 days prior to start date.
