Serves as a primary point of contact for NICU parents, caregivers, and referring physicians to access care services. Supports and manages the day- to- day care of identified infants in collaboration with their families and the NICU team of healthcare professionals to ensure timely, high-quality care. Serves as the clinical liaison for new and existing patients. Ensures seamless access to necessary care through assessment of the infant's and family's financial, emotional, social, and clinical needs throughout the NICU trajectory and connects the infant and family to needed resources. Assists in the development and implementation of patient care protocols, pathways and in updating departmental policies and procedures. Coordinates NICU family resources for parent education and support services. Participates in NICU and Children's Hospital community outreach events.
Job Duties
- Functions as a clinical liaison for new and existing patients
- Works collaboratively with the NICU Team to develop the patient's plan of care.
- Assesses infants and families for post-discharge need; informs parents on discharge planning options based on diagnoses; prognosis, and available resources prior to discharge or transfer.
- Collaborates with multidisciplinary teams to achieve the highest level of coordination and communication across teams to ensure the preparedness of the infant and family prior to discharge or transfer
- Maintains effective communication with providers, nursing staff, lactation consultants, therapy services, families and others related to discharge planning
- Assists in the development and implementation of educational materials.
- Provides information, answers healthcare-related questions, and refers NICU patients to resources both within the health network and the community.
- Gives supportive resources to the patient and the families to promote a positive outlook.
- Acts as Liaison for Today is a Good Day NICU support organization
- Acts as patient liaison and facilitates communication between patient, family, and Health Care Providers.
Minimum Qualifications
- Bachelor's Degree Nursing
- 3 years NICU RN experience
- Ability to advocate for patients with all members of the health care team.
- Excellent communcation skills
- Ability to manage a clinical caseload of patients.
- Ability to obtain and interpret information in terms of the patients needs that are age appropriate.
- Skilled in psycho-social assessment
- Current knowledge of clinical practices and related information systems and computer applications.
- Knowledge of community resources and how to access them efficiently and effectively.
- Knowledge of patient education techniques and principles
- Proficiency in Microsoft Office Applications
- Registered Nurse Upon Hire
- Health Services\BCLS - Basic Life Support Upon Hire
- CN-NEONATL - Certified Neonatal RN
Preferred Qualifications
- Master's Degree Nursing or related field
- Bilingual skills (Spanish and English)
Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
