Job Title: Orthopedic Nurse Navigator
Work Environment and Schedule: The Orthopedic Nurse Navigator position can be full time or part time. It can also be done remotely, in-person or with a hybrid schedule.
Job Summary: The Orthopedic Nurse Navigator plays a key role on the interdisciplinary team as the single point of contact to coordinate care both in and outside the hospital setting for patients having total joint replacement surgery. They focus on patient advocacy, care coordination, disease specific education and data management to improve outcomes and patient satisfaction.
Duties/Responsibilities:
1. Provide expertise as a professional Registered Nurse with orthopedic clinical knowledge
2. Coordinate care as the single point of contact/resource for patients, staff and physicians
3. Act as a patient advocate and educator by providing education to patients, families, and significant others; act as an information resource to students, health care professionals, patients, and the public.
4. Collect and report data from patients on barriers, health care, medications, prior treatments, etc.
5. Track order completion and follow up. Monitor patient compliance with patient treatment plan.
6. Assist patients with referrals to outside resources and care coordination as needed.

Education and Experience:
Registered Nurse
Orthopedic Experience
Current CPR Certification
