The primary role of the Nurse Navigator is to:
- function as a health advocate to aid individuals in receiving the physical health and behavioral health care
- assist in coordinating medical and behavioral health appointments, hospital care and discharge to the community
- arranging travel to appointments
- development of personal plans for consumer health collaboration and wellness.
Earn $33.15 / Hour.

Requirements
The ideal candidate will possess the following qualifications:
- Licensed as a Registered Nurse in Pennsylvania
- Currently meets all requirements per the state nursing board
- Minimum one year experience working with individuals who have mental health diagnoses
- Experience with addictive diseases and/or community mental health preferred
