Next Step Case Manager

General Overview:
This position is responsible for empowering our clients to accomplish their goals as it pertains to ending their homelessness and retaining housing. The Next Step Case Manager (NSCM) receives clients already assessed by the Intake Case Manager. The NCM will prioritize client needs by turning them into steps, help them take each step by leaning on other community resources, meet direct needs to fill any community service gaps, and ultimately help clients continue to progress towards housing. After being housed, the NCM will maintain the relationship to help with housing retention.
Responsibilities and Duties:
Case Management
- Receive cases from the Intake Case Manager and complete a screening to identify if the candidate will benefit from case management services.
- Conduct motivational interviews with clients to determine their current resources and goals, as it pertains to helping them obtain housing, shelter, and/or employment.
- Assist each client with creating a plan, prioritizing their needs by turning them into steps to help each client accomplish their goals.
- Empower them to pursue their goals as it pertains to obtaining housing (or employment) and hold them accountable to those goals.
Referrals
- Keep and monitor a log of each organization in Central Florida that supports the sheltered and unsheltered. Update the log as needed on a monthly basis.
- Refer clients to other organizations to obtain services and resources we do not offer.
- Hold clients accountable to referrals and follow up with clients, systems, or organizations to confirm referral was followed through.
- Utilize budget to fill service or resource gaps if there are no organizations that will meet a need that a particular client may currently have.
Documentation
- Keep detailed internal notes for each client in a secure location. Will ensure privacy and confidentiality of all notes taken.
- Obtain signed Authorizations to Obtain and Disclose Information before entering info in HMIS if not already obtained.
- Documentation will be done through the Homeless Management Information System (HMIS) to support the Continuum of Care (COC) coordinated effort to end homelessness. All referrals will also be documented in HMIS.
Supervision
- Assist in supervising up to two social work interns as directed by the Director of Care Services.
- Assist in interviewing and orientation of new social work interns.
- Assist in conducting training with social work interns as a group and individually.
- Be an ongoing resource and coach for interns throughout their internship experience.

Qualifications:
Requirements
- Must have a Bachelors in Social Work (BSW) or equivalent degree
- Excellent verbal and written communication skills
- Strong problem solving and interpersonal skills
- Organized accurate, thorough, detail-oriented and able to monitor work for quality
- Intermediate level with MS Office Suite and Google Docs
Preferences
- Experience in working with the homeless population
Physical Requirements
- Ability to lift up to 50 pounds
- Ability to stand, walk, sprint, run and patrol for extended periods
- Must hear and see well (either naturally or with correction)
- Must be able to bend, stoop, squat, kneel, push, pull, and reach overhead
- A considerable amount of time making repetitive motions
- Work in a fast-paced environment with frequent interactions with customers
- Ability to function with frequent temperature changes between indoors and out
- Work in various weather conditions, including heat, cold, and rain
- Stand and walk continuously for up to 8-10 hours during a shift while providing customer care
- Considerable amount of time using a telephone and/or computer
- Sit at a desk and use a computer for extended periods to perform data entry and
administrative tasks
- Answer phone calls and communicate with colleagues and clients verbally throughout
the day
- Use hands and fingers for typing and operating office equipment, such as printers
and scanners
- Use hands and fingers for precise mouse movements and keyboard shortcuts
- Review and analyze printed materials for accuracy and quality
- Attend meetings and collaborate with team members in various office settings
- Collaborate with colleagues and communicate professionally in person and via email
- Sit at a workstation and use a computer to respond to customer inquiries and resolve
issues
- Use a headset to communicate with customers over the phone for extended periods
- Navigate between multiple software systems to access customer information
- Occasionally stand and walk within the office to consult with supervisors or assist
colleagues
- Attend meetings and collaborate with team members when needed
