Job Description
Do you have experience in public health? Are you ready to work with Utah's thirteen local health departments (LHDs), non-profits, healthcare systems and other partners to prevent type 2 diabetes in Utah? If this is you, then The Office of Health Promotion and Prevention (OHPP) within the Utah Department of Health and Human Services (DHHS) needs you to as a National Diabetes Health Program Specialist. In this role, you will be assigned to support the Healthy Environments Active Living (HEAL) Program carrying out activities related to Diabetes Prevention.
The HEAL Program's goals include preventing obesity, diabetes, and heart disease and stroke and reducing health disparities through policy, system and environmental changes. The incumbent will support multiple projects related to type 2 Diabetes Prevention and will rely on extensive collaboration with partners and stakeholders. You will work primarily with federal grants from the Centers for Disease Control and Prevention (CDC) that emphasize chronic disease prevention and management.
You will coordinate and facilitate the Steering Committee for the Prevention of Diabetes, a collaborative of Utah's LHDs, non-profits, and other organizations. Additionally, you will support contractors across the state to achieve specific outcomes related to Diabetes Prevention. Ultimately, your work will help reduce the number of people with Type 2 diabetes throughout the state.
This position involves facilitating large groups of public and private partners, developing and executing quality improvement initiatives in health systems and clinics, and supporting the National Diabetes Prevention Program through a variety of initiatives, campaigns, and partnerships. You will make a difference from day one!
Notes:
  • Occasional travel for this position is required.
  • Remote work is available for this position but you must be able to work in-office at least two days per week and as otherwise needed/required.
  • With the exception of official State of Utah Business or with prior approval through proper channels, all work must be physically completed from within State of Utah boundaries.
  • Current funding for this position runs through July 2024. This position may be extended / become permanent based on Agency need.

Principle Duties:
As our National Diabetes Health Program Specialist, you will:
  • Facilitate the Steering Committee for the Prevention of Diabetes to develop streamlined, cohesive and impactful statewide efforts to prevent diabetes.
  • Provide training and technical support to LHDs around prediabetes and the National Diabetes Prevention Program.
  • Identify new and innovative opportunities to prevent diabetes and initiate new statewide efforts.
  • Conduct outreach to public, private and non-profit partner organizations.
  • Support data collection relevant to activities.
  • Represent the HEAL Program and DHHS in the community.
  • Develop and maintain partnerships with organizations that share similar goals.
  • May serve as a liaison between HEAL and LHDs, duties of which include supporting LHDs with monthly and annual reporting, assisting with contract negotiations, onboarding and training new staff, and serving as a resource for questions from LHDs.

  • At least a Bachelor's degree in public health, health education.
  • A strong public speaking ability and is comfortable facilitating discussions with groups of varying backgrounds.
  • Ability to manage multiple projects at once and adhere to quick turnarounds.
  • Comfortable with abstract projects and soft deadlines.
  • Willing to initiate contact and collaborate with other organizations.
  • Able to work well on a team / various teams.
  • Able to explain complex information concisely and accurately.
  • The ability to organize and summarize evaluation information in a clear and concise manner.
  • The ability to create or update public health information in plain language to be read by the public.
  • Familiarity with principles of health equity, social determinants of health, etc.
  • Highly organized, detail-oriented and self-motivated

Preference May Be Given to Applicants That Have/Are:
  • At least one year of experience managing, coordinating, or supporting public health programs or initiatives, ideally related to chronic conditions.
  • At least one year of experience convening with partners from multiple organizations.
  • At least two years facilitating various groups.
  • At least one year of experience managing, coordinating, or supporting public health programs or initiatives, ideally related to chronic conditions.
  • At least two years of experience working with Public Health or other related field areas.
  • At least a Master's Degree in public health, health education, or related field.
  • A trained lifestyle coach and facilitated with at least one full cohort of the National Diabetes Prevention Program (NDPP).
  • Bilingual experience. Spanish as a second language is a BIG plus!

Working Conditions:
  • The typical office setting for this job is adequately lighted, heated and ventilated. However, you may encounter some risks that are associated with any office setting, for example, the safe use of office equipment, avoiding trips and falls, observing fire regulations, etc.
  • Standard Schedule: Monday - Friday, 8:00 am to 5:00 pm. The schedule can be flexed based on work section coordination and approval.

Physical Requirements:
  • Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
