Job Description
Plans and implements specific treatment programs for patients according to the principles and practices of Physical Therapy. Completion of an accredited Physical Therapy program.PT licensure in the state of Montana BLS Certified, Valid driver's license. Receives physical therapy orders for Home Health/Hospice through the appropriate channels. Evaluate patients and establish a written plan of treatment with short and long term goals. Treat patient within established physical therapy guidelines.
Keep appropriate documentation records (clinical notes, progress notes, discharge notes, orders etc.) Documents time appropriately Supervise physical therapy assistants in accordance w/CMS and state requirements. Maintain open lines of communication within department, between departments and with the physicians.
Provide appropriate in services to staff members as requested Completes chart reviews and clinical documentation in timely manner 10. Completes Oasis SOC, recertification's, resumption of care and discharges per CMS guidelines. Completes other duties as assigned.
