RADIAS Health is hiring a Mental Health Practitioner to join our Housing Navigation Program in providing services to individuals experiencing long term homelessness who have mental illness and/or substance use disorders. The Housing Navigation program helps clients find their own independent housing within Ramsey County and provides supportive services to help them maintain their housing.
The Mental Health Practitioner leads the housing search and provides support and advocacy to overcome barriers affecting housing in the past. In this role, you will provide outreach, housing transition, and tenancy sustaining services to vulnerable adults. The primary function of the practitioner is to assist the individuals served in restoring skills essential for managing his or her illness, treatment, in order to support ongoing tenancy in independent.
Hours: Monday - Friday 8 am - 4 pm or 9 am - 5 pm, 40 hours per week
Duties and Responsibilities:
Provide instruction and monitoring on the identified service areas indicated on the client’s Diagnostic Assessment, other assessments, and other recommendations by client’s treatment team.
Decide on appropriate interventions or assist the client in obtaining and maintaining the highest level of independence possible.
Continually monitors client progress and with the clinical team and modifies the Housing Support Plan and Housing Support Crisis Plan as appropriate. Practitioner communicates changes in service needs and recommendations to Clinical Supervisor/Mental Health Professional, Case Manager, and rest of treatment team on an ongoing basis to coordinate client care.
Maintain records of progress for each face-to-face meeting and telephone contact with client. Reports will include an assessment of client’s ability to function and manage the symptoms of his/her illness based on observation and interaction with the client.
Assist clients to create a plan to work towards service goals in regular meetings with the client.
Schedule appointments to see the client to give requested services for the amount of time and frequency agreed upon by the clinical treatment team and client, and as needed based on the client’s immediate needs.
Intervene as necessary for emergencies and/or when he/she assesses that the client is unable to manage his/her illness in a reasonable manner. Contact to appropriate emergency services, doctors, staff, and emergency family contacts will be made to alert them of intervention planned or taken.
Coordinate and cooperate with other agency and community providers to provide the best possible care for the client, including but not limited to landlords and other community resources. Practitioner will complete all necessary Release of Information documents for communication with other providers and keep these current for each client.
Maintain records of contact with other providers regarding client care.
Meet regularly with Program Director for individual and group reviews of client needs and receive direction for treatment.
Assist client in obtaining and maintaining benefits including housing subsidies and other economic assistance, as well as providing assistance in budgeting monthly funds.
Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
Participate actively in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
Must be able to transport clients in the community.
Requirements
Candidate will have a Bachelor's degree in Psychology or Social Work plus 2,000 hours work experience providing services to adults diagnosed with a serious mental illness

No college degree and 4,000 hours supervised work experience in the delivery of services to adults or children with mental illness.
Must have car, auto insurance and driver's license.
