Position Overview:
Under the supervision of the Access Center Program Coordinator, performs screenings, eligibility assessments, coordinates care with community agencies, assists with referrals, and may provide short term crisis stabilization services as needed. Services are provided to adults, adolescents, and children who are homeless. Services are provided at local homeless shelters, community programs that serve the homeless population, the CMH office, and other locations, as necessary.
This is a full-time benefited position working primarily in the community and at the James Street Campus in Holland MI. Some travel to other county locations, as needed. This position will work Monday-Friday, 8:00 a.m. - 5:00 p.m., with possible work in evening hours, as needed.
Essential Job Functions
The essential functions of this position include, but are not limited to, the following:
  1. Provides screening assessments to individuals experiencing homelessness, including risk assessments, clinical assessments, diagnoses, and eligibility determinations.
  2. Collaborates with community agencies and programs working with the homeless population.
  3. Provides crisis stabilization and on-going brief therapy to address immediate problems or concerns within the homeless population.
  4. Makes referrals to Community Mental Health services or other community services.
  5. Provides consultation to other agencies regarding services and Mental Health issues of consumers.
  6. Assists with case management as needed.
  7. Ensures care coordination between multiple agencies for consumers.
  8. Flexibility in the selection of appropriate intervention modalities for each consumer.
  9. Provides phone screenings and walk-in coverage as needed.
  10. Other duties as assigned.

Required Education, Training and Experience
Master's degree in social work, psychology, or a related Mental Health field is required. Two years of relevant professional experience in a Mental Health agency preferred. Lived experiences with mental illness, developmental disabilities, or substance use disorders are valued.
Licenses and Certifications:
Must possess or obtain State of Michigan licensure as a Licensed Master's Social Worker, Limited Licensed Master's Social Worker, Licensed Psychologist, Limited Licensed Psychologist, Licensed Professional Counselor, Limited Licensed Counselors, or Temporary Limited License Psychologist. Additional certification as a CADC, CAADC and CCS are preferred. Candidates who are close to obtaining the required licensing may be considered.
Additional Requirements and Information
Required Knowledge and Skills:
  1. Thorough working knowledge of DSM 5.
  2. Thorough working knowledge of structured assessment tools such as the LOCUS, C-SSRS, and CAFAS.
  3. Thorough working knowledge of the Michigan Mental Health Code.
  4. Thorough working knowledge of varied therapeutic interventions.
