Responsibilities:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop individualized service plans
- Coordinate and collaborate with community resources to ensure clients receive appropriate support
- Assist clients in accessing necessary services such as healthcare, housing, and employment
- Advocate for clients' rights and ensure their voices are heard in decision-making processes
- Monitor and evaluate client progress towards goals and make adjustments to service plans as needed
- Provide crisis management and intervention when necessary
- Collaborate with interdisciplinary teams to provide comprehensive care
Skills:
- Strong crisis management and intervention skills
- Knowledge of behavioral health principles and practices
- Familiarity with Medicare, child welfare, Section 8, and workforce development programs
- Experience in mentoring and providing support to individuals of diverse backgrounds
- Proficiency in motivational interviewing techniques
- Ability to navigate complex systems and connect clients with appropriate resources
- Excellent communication and interpersonal skills
Note: Experience or knowledge in childcare is a plus but not required for this position.
