The Memory Care Director (LPN) provides assistance to residents, families, and caregivers in a manner that exceeds community service expectations. Familiarity and willingness to work with elderly population is essential for this role, including those with dementia or memory loss.
ESSENTIAL POSITION FUNCTIONS
- Ensure completion of appropriate resident assessment & related documentation for each Memory Care resident before, during, and after move-in.
- Complete appropriate social history documentation for all residents considered for Memory Care before, during, and after move-in.
- Ensures specialized programming services are delivered to Memory Care residents and appropriate documentation for same is completed timely.
- Recruits, trains, and supervises Memory Care activity staff to implement group activities as well as specialized programming & services to Memory Care residents.
- Trains Memory Care staff to implement specialized programming & services to Memory Care residents.
- Collaborates with Wellness Director to develop and maintain appropriate documentation and work flow for Memory Care.
- Maintains residents’ records and documentation of services.
- Facilitates an environment that promotes the optimal well-being and maintains the highest level of functioning for each resident.
- Ensures the needs of each resident are met according to their individualized service plan.
- Provides orientation to new residents, families, and employees regarding Memory Care.
- Collaborates with the Wellness Director to ensure Memory Care employees are appropriately educated and trained.
- Partners with the Wellness Director to address health concerns of Memory Care residents.
- Communicates with families and/or responsible party regarding Memory Care services, specialized programming, and the overall well-being of residents residing in Memory Care.
- Develops and implements monthly activity calendar for Memory Care residents based on their varied backgrounds, needs, and enjoyment.
- Coordinates and participants in the monthly Community Specific support group.
- Facilitates the involvement of resident families, members of the community and volunteers in activities and events.
- Actively seeks opportunities for professional development.
- Maintains the Memory Care common spaces in a neat and orderly manner.
- Serves on the Community’s Safety Committee.
- Other duties as assigned
SUPERVISORY RESPONSIBILITIES
- Directly supervises the Memory Care activity staff. Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interview, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
- Collaboratively supervises and trains Memory Care caregivers, along with the Wellness Director, working with Memory Care residents. Responsibilities include training employees; planning, assigning, and directing work. In partnership with the Wellness Director in appraising performance, rewarding, and disciplining employees; addressing complaints and resolving problems.
- Rotates on-call supervisory duties as determined by the Executive Director to satisfy the needs of the Community.
- Serves in rotation as Weekend Manager on Duty.
SPECIAL REQUIREMENTS
- Must be willing work flexible hours, including some evenings and weekends due to special events.
Education and Qualifications
- To perform this position successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- Preferred Associates or Bachelor Degree in a human services related field.
- Minimum of one to three years’ experience working with residents with memory loss in a long term care environment or assisted living community.
- Minimum of one-year experience of serving in a lead or supervisory position.
- LPN preferred.
