Seeking Member Connections Representatives to join our team!
Description:
Assist the health plan in locating members and to help them in reapplying for Medicaid. Complete Health Risk Assessment and explain benefits of My Health Pays. Address social determinants of health and engage into Care Management.
Educate members on health care benefits through home visits and telephonic outreach
Assist members with activities related to accessing care
Document information and activities in applicable systems
Schedule and complete home visits for noncompliance, benefits information, ED and readmission diversion and other high risk issues
Conduct targeted phone calls for compliance, HEDIS initiatives and health risk assessments
Follow-up with appropriate parties on status of high risk member referrals
Assist in finding and connecting members to community social services and agencies
Ability to travel throughout designated region
Additional Skills & Qualifications:
Any combine experience below and/or Bachelor's degree in disciplines including social science, psychology, sociology, social work, or related field

Preference to individuals whom are bilingual, but not required
