We are seeking highly organized, proactive, compassionate individuals to join our team as Member CARE Advocates to support our members, their families, and our clinical team.
As a Member CARE Advocate, you will assist in identifying challenges being experienced by our members and work directly with those individuals, as well as a myriad of local and national resources, to elevate or eliminate those instances. We partner with our Gloria Gates CARE healthcare team to address the needs of members and their families, assisting with member-facing duties as needed. Success in this initiative necessitates the development of a profound level of trust with our members, understanding their healthcare needs and engaging them on how we can best support them and their families.

KEY RESPONSIBILIES:
- Development and fostering of trusting relationships with members and their families.
- Compassionate, thorough guidance and assistance to members and their families relating to all components of their healthcare and socioeconomic needs.
- Assist members to complete necessary application processes to receive assistive services.
- Accurate documentation of member advocacy information/actions in the medical record.
- Excellent working relations with members, visitors, and staff.
- Provision of referrals for services to community/national agencies as appropriate.
- Excellent collaboration with clinical personnel.
- Development and implementation of culturally appropriate community-based programs and outreach events.
EXPECTATIONS:

- Valid Driver’s License and reliable transportation.
- Willingness to travel in the community up to 20%.
- Ability to work independently and within a team.
- Basic computer skills.
- Proficiency in Microsoft office software and communication tools.
- Strong interpersonal and communication skills.
- Ability to multitask and prioritize effectively in a fast-paced environment.
- Maintain confidentiality and protect the practice by abiding by all laws and principles related to confidentiality; keep information concerning practice operations, members, their families, and staff confidential.
- Successful completion of background check and drug screen.
- Maintain compliance with all Gloria Gates CARE standards, including Conduct, Compliance, and all other policies and procedures.
EDUCATION AND EXPERIENCE:
- High school diploma or GED equivalent.
- Previous experience in healthcare or related industries is a plus.
