Job description

JOIN TEAM TRILOGY:
Provides and is responsible for assisting in the administering of medications to residents as ordered by the attending physician, under the direction of the attending physician, the nurse supervisor/charge nurse and the Director of Health Services in a Home for The Aged. The administration of medications shall be in accordance with established nursing standards, the policies, procedures and practices of this facility, and the requirements of this state.
Duties and Responsibilities:
  • Administers medications to residents following the policies and procedures governing the Assisted Living Center.
  • Performs administrative requirements, such as completing necessary forms, charges, reports, etc., and submits as may be required.
  • Accurately and safely prepares, administers, and documents the oral and/or topical medications that are commonly used in an Assisted Living Center and that may be ordered for a resident's use by the attending physician or the Medical Director.
  • Verifies that any medications brought into the Assisted Living are by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy or Pharmacist.
  • Follows and adheres to the labeling policies and practices of the Assisted Living Center.
  • Ensures that all medications administered are properly labeled.
  • Performs medication charting as directed by the Nurse Supervisor/Charge Nurse. Follows Assisted Living procedures in regards to charting of medications.
  • Follows Assisted Living policies and procedures regarding the destruction of medications; assists in carrying out these processes under the direction of the Assisted Living Pharmacist or the Director of Health Services, including documentation requirements.
  • Assists in documenting and removing medications that are discontinued by the attending physician.
  • Follows Assisted Living procedures governing drug receipt errors, including notifying the delivery source and the Director of Health Services.
  • Ensures that medication is returned and reordered; except emergency drugs.
  • Ensures that documentation of the drug receipt error includes a written error report, signed by an observer/witness and the Medication Technician.
  • Adheres to Assisted Living procedures in holding medications for residents who are transferred from the facility or when the attending physician discontinues a medication.
  • Accurately measures, records, and reports the vital signs of residents.
  • Observes and verifies that medication is ingested or applied as directed. Documents any instance where prescribed medication is not administered, including reason(s) for refusal.
  • Promptly notifies the Nurse Supervisor/Charge Nurse or the Director of Health Services of any errors or reactions to medications by residents.
  • Follows the medication card practices of the Assisted Living Center.
  • Ensures that each medication is administered with a written, signed, dated order from an attending licensed physician.
  • Adheres to the Assisted Living procedure for recording the order in the resident's clinical record, including the type, route, dosage, frequency and strength of each medication.
  • Verifies the identity of the resident before administering the medication treatment.
  • Follows the Assisted Living procedures allowing residents to self-administer their medications. Assists the Nurse Supervisor/Charge Nurse in monitoring and documenting all aspects of such self-administering. Promptly notifies the Nurse Supervisor/Charge Nurse of any failure of a resident to properly take any self-administered" medications.
  • Adheres to established guidelines concerning the storage of drugs and biologicals.
  • Recognizes and is familiar with the meaning and usage of common medical abbreviations, symbols, and terms relative to the administration of medications as used by the health campus, pharmacists, and physicians.
  • Participates in the development and implementation of the resident's comprehensive assessment as necessary.
  • Minimal travel with overnight stay as necessary.
WHAT WE'RE LOOKING FOR:
  • High School diploma or equivalent. Successful completion of AFC/HFA Medication Administration Course.

Licenses/Certifications:
  • Completion of AFC/HFA Medication Administration Course.
  • Valid CPR certification.

Experience:
  • One (1) to two (2) years of HFA Medication Technician experience preferred. Healthcare, senior living or long-term care experience preferred.
  • Exemplary computer skills that include knowledge of the Microsoft Office Suite of products.
