Job Description
Position Summary:
  • To support the Population Health and Quality workflows and provide direct telephonic patient care to support overall quality of Comprehensive Medication Management services and Pharmacy related quality metrics for our attributed patient population.

Essential Job functions:
  • Interact with all levels of medical staff to discuss relevant patient information as warranted.
  • Conduct chart review and patient interviews to identify patient specific reasons for poor medication adherence and resolve identified problem per protocol.
  • Perform medication reconciliation for patients recently discharged from the hospital or as requested by the Provider or Nurse Care Manager per protocol. Record medication information into the "Meds" in the electronic health record and update existing information as needed to ensure accuracy of the patient home medication list.
  • Gather data and generate reports to improve clinical pharmacy quality metrics.
  • Document patient cases in electronic medical record effectively.
  • Appropriately identify situations for referral to Provider or Nurse Care Manager (i.e. medication adverse event, medication ineffective, drug-drug interaction, lab test indicated).
  • Reinforce patient self-management of medications through provision of education on proper medication administration, medication adherence, storage, and drug disposal.
  • Prepare and manage patient education materials/handouts.
  • Make outreach to patients identified from payer medication adherence-related reports.
  • Participate in training and orientation of new employees as needed.
  • Comply with the Summit Health Management incident reporting policy and procedures.
  • Adhere to all Summit Health Management training and accomplishments as required per policy.
  • Provide other job duties as assigned.
  • Report issues and concerns to manager in a timely manner.
  • Based on chart review or patient interview, identify patient specific reasons for poor medication adherence and take appropriate action to resolve problem.

  • Contributes to smooth workflow and exhibits flexibility by properly observing lunch and break periods. Complies and encourages compliance with SHM policy regarding appropriate use of assigned electronic resources (email, internet, etc). Complies and encourages compliance with SHM policy limiting personal use of cell phone, including text messaging (only at meal times, on breaks and outside of patient areas). Complies and encourages compliance with all other SHM and departmental policies, procedures and standards, for example infection control and no solicitation policies.
  • Complies /encourages compliance with all OSHA requirements, including the use of proper body mechanics during all activities and duties. Complies /encourages compliance with all HIPAA policies and procedures including appropriately maintaining confidentiality (including computer screens, public discussions, unattended records, etc.) and releases confidential information only when appropriate. Complies and encourages compliance with all other regulatory policies, procedures and standards.
  • Demonstrates the ability to set goals, assign priorities, anticipate problems, and achieve results, schedules and uses time efficiently, accepts full responsibility for projects and duties assigned, completes assignments on time.
  • Attends and participates in available educational and training events, departmental meetings, employee forums, committees or meetings, as appropriate.
  • Supports & is committed to the goals of the organization, including the delivery high-quality and patient-focused care, and being mindful of efficient operations and expense control.
  • Observes scheduled work hours, works assignments and the Attendance and Tardiness Policy.

Education, Certification, Computer and Training Requirements:
  • High School Graduate/GED or Equivalent Work Experience required
  • Bachelor's Degree Preferred
  • Certified Pharmacy Technician (CPhT) with PTCB required
  • Minimum 3 years of retail pharmacy practice
  • Oregon Board of Pharmacy Technician Registration required
  • Ability to communicate in English, both orally and in writing
  • Use of standard office equipment (phone, fax, copy machine, scanner, email/voicemail)
  • Use of standard office technology in a window based environment

Travel:
  • limited travel to satellite locations as needed
