Uses crisis intervention, problem solving model, community organization, and advocacy skills in identifying needs and resources in the hospital and community.
Certification in Case Management (CCM, ACM RN-C) required within one year of eligibility. BLS required within 90 days of hire.
Master's Required.

- Strong verbal and written communication skills are crucial for conducting educational sessions, collaborating with diverse teams, and advocating Palliative Care.
- The ability to empathize with patients, families, and healthcare providers is vital for fostering a supportive and collaborative environment.
- Experience in designing and delivering educational programs, workshops, and training sessions.
- Understanding of adult learning principles to tailor educational content effectively.
- Familiarity with medical coding practices, particularly in the context of Palliative Care services.
- Ability to implement and manage changes smoothly, utilizing change management techniques to overcome resistance.
- Strong problem-solving skills to identify barriers to utilization and develop effective solutions.
- Proactive and self-directed, taking the lead in initiating committees or projects to enhance ACP efforts.
