Form of workGENERAL SUMMARYThe Medical Social Worker will provide assessment, advocacy, crisis interventions and mental health support toadults 18 years of age and older in a clinical setting. The Medical Social Worker will possess knowledge,compassion and skills to identify and successfully connect patients to resources within the community thattargets social determinants of health. The Medical Social Worker will possess problem-solving skills inassisting patients navigating the health care system.
Medical Social Worker will be an integrate part of a multidisciplinaryteam aiding in proficiency communicating patients' needs and resources. This position will requirethe Medical Social Worker to float to other clinics outside of their designated area. This position will require aSocial Worker to apply critical thinking skills, effective communication amongst patients, community partners,staff and providers of Cooper Green Mercy Health Authority Services.KEY RESPONSIBILITIES Mentor Bachelor and Master level social work and apprentice students.
Provide direct social work practice, to patients and their families facing social economic barriers and stressors Collaborate with Patient Assistance Program to ensure patients are receiving and in compliance with criteriaof program Consistently protects patients privacy, rights and confidentiality Abide by the ethical principles outlined in the NASW Code of Ethics Identify, implement, new and creative ideas for resources within CGMHSA while collaborating withcommunity partners and other duties as assigned to support facility operations.CRITICAL KNOWLEDGE/SKILLS Strong communication and written skills a must. Knowledge of crisis intervention and response Knowledge of Microsoft office to include word, excel, power point Referrals, networking, time management and conflict resolution
