JOB SUMMARY
Under the direction of the Director of Nursing, the Medical Records Designee is responsible for; initiating and maintain Medical Records for the Health Care Facility residents in accordance with State and Federal regulations. Health records shall also be initiated and maintained for assisted living and residential residents.
ESSENTIAL FUNCTIONS
Include the following. Other duties may be assigned as necessary.
· Provides quality Customer Service efficiently to residents, families, co-workers and vendors in a manner to ensure satisfaction.
· Responsible for initiating, maintaining and auditing Medical Records to meet State and Federal standards.
· Maintains accurate log of admissions, transfers and discharges.
· Codes and indexes all disease entities for each resident on admission, throughout stay and on discharge. Keeps all records current.
· Completes quality audits and participates in quality assurance monitoring as part of the facility QA program.
· Assures confidentiality of all medical record information.
· Performs all duties in a safe and efficient manner. Wears safety equipment as required for the job. Reports any safety hazards and/or accidents to supervisor.
· Attends all mandatory in-service meetings. Complies with all department and facilities policies and procedures.
JOB REQUIREMENTS and QUALIFICATIONS

· Two years’ experience in admissions/Medical Records in a long-term care setting and computer literacy.
· Sufficient verbal and written communication skills to perform the tasks required.
· Basic mathematical skills required.
· Demonstrates ability to understand and follow State and Federal regulations and established policies and procedures of the facility; plans, organizes and prioritizes work assignments efficiently. Functions efficiently and effectively with minimum direction.
· Self-starter, detail oriented and strong interpersonal skills.
