MEDICAL CASE MANAGER

(Bangor)


Note: With an aim to provide top-quality service to clients and communities and better our work-life balance and overall well-being, HEAL operates a 4-day workweek. All staff have 3 consecutive days off each week. We do this to provide better opportunities for self-care, time to rejuvenate, and more time with family and friends so we are fresh, present, and focused on the needs of the people and communities we serve. HEAL considers a 4-day workweek as fulltime for benefits and staff are compensated for the equivalent of working 5 days each week.


JOB SUMMARY

Responsible for the provision of primary case management services to persons living with HIV/AIDS. The Case Manager shall work closely with persons diagnosed with HIV/AIDS to provide Medical Case management, adherence counseling, and support services. ESSENTIAL FUNCTIONS

  • Maintain a caseload of persons living with HIV/AIDS while building a collaborative relationship with both the clients, providers, and other community members drawing upon knowledge and skills in mental health, substance use, addictive behavior, grief and bereavement, trauma recovery, and family systems.
  • Meet with clients to identify what needs they have and how the quality of their lives can be improved; determine the case management services that would best meet these needs; develop goals and objectives that will address these needs, and document these in the care plan. Review the progress towards these goals every quarter.
  • Provide referral and linkage services to people living with HIV/AIDS. Advocate and collaborate with other service providers in the provision of services on behalf of clients.
  • Work with clients to achieve client-centered goals while maintaining ethical boundaries.
  • Complete documentation on a weekly, quarterly, semi-annual, and annual basis according to MaineCare and Ryan White grant requirements. Case notes and billing units must be captured in CAREWare within 72 hours of client encounter.
  • Responsible for achieving monthly billing goals, maintaining a high standard for documentation and data integrity, and maintaining exceptional standards of client services per Ryan White and Medicaid guidelines.
  • Uphold the mission and integrity of client-centered case management and maintain ethical standards and continuing education relevant to the role and social services profession.
  • Participate in annual program planning, including goal setting for the existing program and developing new projects.

SKILLS AND QUALIFICATIONS

  • Experience with case management and/or human service; familiarity with community resources.
  • Bachelor’s degree in Social Work, Psychology, Sociology, or Human Development.
  • Demonstrated commitment to and appreciation for Inclusion, Diversity, Equity, and Accessibility (IDEA).
  • Experience of at least two years in providing direct services to persons through social service, mental health, or substance use agency preferred.
  • Familiarity with persons living with HIV/AIDS, and the barriers and challenges they encounter, are preferred.

WORK ENVIRONMENT AND OTHER REQUIREMENTS

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 30 pounds.
  • COVID-19 vaccination required.
  • Access to reliable transportation that has full coverage insurance.
  • Ability to work remotely occasionally based on workload.
