Position Overview
Responsibilities:
- Collaborate with healthcare providers to facilitate the implementation of RPM solutions.
- Assist in the setup and maintenance of remote monitoring devices.
- Monitor patient data and escalate any issues or abnormalities to the healthcare team.
- Engage with patients to ensure adherence to remote monitoring protocols.
- Medication Reconciliation (Med Rec):
- Conduct comprehensive medication reviews for patients.
- Collaborate with pharmacies and healthcare providers to ensure accurate and up-to-date medication lists.
- Educate patients on medication management and adherence.
- Documentation and Communication:
- Maintain accurate and detailed records of patient interactions and interventions.
- Communicate effectively with healthcare professionals, patients, and internal team members.
- Remote Work Collaboration:
- Participate in virtual meetings and collaborations with the DayaMed team.
- Utilize remote communication tools to stay connected and engaged.

Qualifications:
- Certified Medical Assistant
- Strong communication and interpersonal skills.
- Ability to work independently and efficiently in a remote work environment.
