Position Summary
- Sign Admission Agreement with new residents/or responsible parties
- Explain Insurance coverage to new residents eg: Co-pays/Deductibles/Out of Pocket Expenses
- Screen, interview, and assess potential residents/current residents and family members to determine Nursing Home Medicaid eligibility
- Attend UR Meeting with Social Work, MDS and Rehab Team to determine which residents needs to be picked up on Medicaid
- Have weekly/Bi-weekly meeting with biller/tracker
- Attend quarterly Aging meeting with Administration/Board
- Work with Social Security Office to obtain income information and submit Rep Payee Information
- Request, Collect and Prepare all the necessary documents to apply for Nursing Home Medicaid
- Upload documents to be sent electronically to HRA
- Go over NAMI information with Resident and Family
- Collect NAMI (Social Security & Pension income)
- Process Re-certifications on time to maintain Medicaid Roster
- Correspond with HRA Case Workers on Ongoing Medicaid applications to determine status, and answer all reconsiderations
- Follow up to ensure that all applications are approved
- Secure and complete appropriate MAP forms in accordance with request from Medicaid eg: MAP 2159I, MAP 2159W etc.
- Promptly Send Discharge Form to Medicaid when residents are discharged home in the community MAP 259F
- Assist Admission Director/Assistant Director with processing Admissions from the hospital eg: set up resident in Sigma for admission
- Perform other job-related duties as assigned by Director of Admissions
Medicaid Coordinator Requirements
- 3-5 year experience with New York City Nursing Home Medicaid applications
- High School Diploma or Equivalent
Prefers
- Bachelor's Degree in Business, Accounting/Finance, Marketing, Social Work, or a related field
- Previous experience in a Skilled Nursing Facility
