MDS Coordinator at Logan Square Rehab and Healthcare!
We are looking for candidates with experience as an MDS Coordinator for this role
Our skilled nursing facility provides traditional nursing home care as well as sub-acute rehabilitation, hospice, respite, and Alzheimer’s care. Our goal is to achieve the highest practicable physical, mental, and psychological well-being while treating each resident with the utmost dignity and respect.

Requirements
- MDS Coordinator: 1 year of experience required.
- Must possess valid unencumbered, PA State License. LPN Nursing degree from an accredited college or university preferred.
- Knowledge of Medicare and Medicaid regulations and coverage guidelines related to skilled nursing.
