Sign-on Bonus: $12,000Job Description:The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessment process in accordance with regulatory requirements and company policies and procedures. The MDS Coordinator will work closely with the interdisciplinary team to ensure accurate and timely completion of the MDS assessments and care plans. The MDS Coordinator will also be responsible for maintaining accurate and complete resident records and ensuring compliance with all state and federal regulations related to the MDS process.
Key Responsibilities:- Coordinate and complete the MDS assessment process in accordance with regulatory requirements and company policies and procedures
- Work closely with the interdisciplinary team to ensure accurate and timely completion of the MDS assessments and care plans
- Maintain accurate and complete resident records
- Ensure compliance with all state and federal regulations related to the MDS process
- Assist with call coverage for DON
- Current license as a Registered Nurse (RN) in the state of Wisconsin
- Assessment skills necessary
- Computer skills necessary
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
