Century Center for Rehabilitation & Healing is now hiring full time MDS Care Plan Coordinator who is committed to providing the best life and care for our residents. Under the: direction of the Director of Nursing Services, the MDS / Care Plan Coordinator is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning. Century is an 88-bed skilled nursing facility with private and semi-private rooms located in Century, Florida. Our focus is quality care in a supportive and friendly environment. MDS COORDINATOR (RN / LPN) QUALIFICATIONS:

• Organizational Skills/Ability to Multitask
• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) License by the State
• Knowledge of PDPM/Clinical Concepts
• Ability to work with facility nursing management to identify and fix care documentation notes
