Responsibilities:
- Oversee and manage the MDS (Minimum Data Set) process for accurate and timely completion
- Ensure compliance with all state and federal regulations related to MDS assessments
- Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
- Conduct comprehensive assessments of residents' medical conditions and care needs
- Develop individualized care plans based on assessment findings
- Monitor and update care plans as needed to reflect changes in residents' conditions
- Coordinate with healthcare professionals to ensure appropriate medical documentation is obtained and maintained
- Provide education and training to staff on MDS coding and documentation requirements
- Participate in quality improvement initiatives related to resident care

Skills:
- Strong knowledge of health information management principles and practices
- Experience in case management and medical documentation
- Familiarity with physiology, anatomy, and ICD-10 coding
- Proficiency in utilizing electronic health record systems (e.g., Epic)
- Knowledge of utilization review and utilization management processes
