MDS Coordinator

We are currently seeking an experienced and detail-oriented MDS Coordinator to join a Skilled Nursing Facility in Brooklyn N.Y.
Position Overview:
As an MDS Coordinator at you will play a vital role in ensuring accurate and comprehensive assessments of the patients' needs for care planning and reimbursement purposes. The ideal candidate will possess a deep understanding of the Minimum Data Set (MDS) process, exceptional organizational skills, and the ability to collaborate effectively with the healthcare team.
Key Responsibilities:
- Complete and coordinate the Resident Assessment Instrument (RAI) process, including the Minimum Data Set (MDS), Care Area Assessments (CAAs), and Resident Care Plans.
- Ensure accurate and timely submission of MDS assessments in accordance with regulatory guidelines.
- Collaborate with interdisciplinary team members, including nursing, therapy, and social services, to gather information for accurate assessments.
- Participate in the development and implementation of individualized care plans based on MDS assessments.
- Stay informed about changes in regulations related to MDS and maintain compliance with state and federal requirements.
- Provide education and training to staff members on MDS processes and documentation requirements.
Qualifications:
- Current license as a Registered Nurse (RN) in NY.
- Minimum of 1 year of experience as an MDS Coordinator in a skilled nursing or long-term care setting.
- In-depth knowledge of the RAI process, MDS completion, and reimbursement principles.
- Strong understanding of Medicare and Medicaid regulations related to MDS.
- Excellent organizational, communication, and interpersonal skills.
- Proficient in Sigma Care.
Salary:
$115,000 - $120,000
