The Manager, Social Work is the social work leader for specific HbMC markets within an assigned region and division, partnering closely with local Market Leadership team(s) to formulate strategies and generate outcomes. The Manager, Social Work reports directly to a Market Health Services Director, and works closely with all Market Health Services Directors in a region / division based on assigned team. The Manager, Social Work will also partner closely with the Social Care and Behavioral Health Leadership Team(s) to ensure alignment with the national vision and strategy for social work and behavioral health interventions and care management activities.

Primary Responsibilities:
- Plan, organize, and direct the work of master's prepared social workers, inclusive of medical social workers and behavioral health care managers who, as part of a larger HbMC interdisciplinary team, are responsible for assessing and addressing the psychosocial needs of members and providing care management to address issues related to palliative care, behavioral health and social determinants of health
- Improve and refine workflows regarding social work and behavioral health referral processes specific to meet the need of market(s)
- Provide individual supervision to assigned staff across multiple market(s) and region(s) / division
- Enforce and ensure standardization of workflows through routine chart audits and oversight of the delivery of care management interventions to include feedback on objective measures but also the clinical delivery of services.
- Respond to requests for consultation to assigned team members as needed
- Participate as an active member of the local interdisciplinary team (IDT) by joining huddles and other team meetings and participating and leading clinical case discussion
- Participate as an active member during interdisciplinary team (IDT) meetings with the Health Plan and other community provider networks (i.e. IPA, PCP Group, Hospital Group)
- Ensure process for identification and updates to robust inventory of resources for assigned market, region / division to be accessible to all team members
- Participate in interviewing, hiring, evaluating and disciplining of assigned staff
- Assist with education, in-service training and new hire orientation
- Participate in resolution of conflicts between team members, patients and HbMC personnel
- Participate in all leadership meetings as directed by the market, region / division leadership team
- Prepare for and lead regular meetings with applicable team members
- Participate in targeted ride alongs with assigned team members in market(s) and region(s) / division as well as targeted comprehensive chart reviews in order to audit and coach
- Act as the subject matter expert for the market / region for all social work and behavioral health initiatives as directed by the Social Care and Behavioral Health Leadership Team
- Fulfill key role of improving collaboration across team members including those that provide non-clinical patient facing interventions that support the team
- Provide coverage based for applicable assigned team members based on market and region /division needs
- Monitor performance metrics to identify opportunities for process improvements and / or staff education to produce optimal outcomes
- Analyze and understand reports; identify reasons for performance gaps and identify opportunities for improved performance
- Act as a subject matter expert for the delivery of care management interventions
- When appropriate, provide clinical supervision to eligible social workers
- Perform related duties as necessary and other duties as assigned
- Access to reliable transportation required; if you are driving a vehicle, you must comply with all the terms of the Optum Motor Vehicle Safety policy

Required Qualifications:
- LMSW
- Valid and applicable licensure, or license eligibility, in assigned state(s)
- 5 years of social work clinical experience (internship not included) that includes direct care management and/or clinical work with patients
- 2 years of social work supervisory experience
- Experience working in or with healthcare, palliative care and / or behavioral health teams and settings

Preferred Qualifications:
- 2 years as a Licensed Clinical Social Worker (LCSW)
- Experience in delivering home-based or field care management
- Demonstrated ability to collaborate and communicate effectively in an interdisciplinary team setting
- Excellent communication skills and knowledge of accessing community resources
