Are you ready to directly impact the health outcomes of the San Francisco communities? Reporting to the Director of Population Health Management, the Manager of Population Health Management (PHM) is responsible for managing key Population Health Management activities of San Francisco Health Plan (SFHP). You will ensure that programs for intervention and performance improvement programs meet contractual requirements including those of DHCS, DMHC, and NCQA and demonstrate health equity for all SFHP members. Your goal is to ensure SFHP’s PHM program requirements and PHM Service are implemented and operate in accordance with DHCS requirements and continues to improve each year by implementing public health programs that benefit member health. You will lead a high performing team to accomplish these goals, providing them with leadership, coaching, and mentoring.

This is a hybrid position, based in our Downtown San Francisco office.

WHAT YOU WILL DO:

Management/Leadership:

  • Provide effective leadership and supervision to staff members and contractors.
  • Support the development of achievable, relevant departmental goals, objectives, and metrics, and ensure they are used to continually motivate staff and support high individual and department performance.

Population Health Management Program and Service:

  • Review and analyze population and patient data related to clinical care and outcomes to evaluate performance, reveal trends, and identify opportunities.
  • Track Population Health service delivery, inclusive of multi-disciplinary initiatives and care coordination as described by DHCS contract requirements, APLs and PHM strategy and roadmap.
  • Support the operations of PHM programs to include, but not limited to completing the Population Needs Assessment, PHM strategy for NCQA; research and develop DHCS required documents for PHM. 
  • Communicate and synthesize PHM performance measurement results and recommend improvement opportunities to providers, executives, and clinic staff.
  • Develop analytical framework and obtain managed care data.
  • Conduct quantitative and qualitative analyses to evaluate programs.
  • Plan and execute population management approaches to improve HEDIS and other quality measures. 

Policies and Procedures:

  • Develop and update Health Services to include, but not limited PHM Policies and Procedures.
  • Implement DHCS requirements and execute business rules through SFHP operations.
  • Monitor performance of PHM requirements as defined by DHCS.
  • Prepare SFHP Health Services audit readiness by developing processes and gathering required information to support response to regulators

Health Education and Linguistic Services:

  • Oversee execution of Health Education and Cultural Linguistic Programs, in accordance with regulatory and NCQA requirements.

WHAT YOU WILL BRING:

  • Bachelor’s degree in a relevant subject area such as Public Health, Nursing, Health Administration, or Social Work. Master’s degree in a relevant subject area such as Public Health, Health Administration, Public Administration, Public Policy, or a closely related field or equivalent work experience required.
  • Minimum 3 to 5 years of experience managing staff
  • Minimum two to three years of experience in health related quality improvement, large scale projects, direct service health care environment, or Population Health programs
  • Experience in working with DHCS or NCQA preferred
