Position Summary
The Manager of Community Health is responsible for the administrative and programmatic oversight of El Centro’s outreach and community health worker staff and initiatives, which directly address non-medical drivers of health. This position is also responsible for the coordination of required services, community involvement, and strategic planning to ensure accurate support for the El Centro’s clinical and support programs.
Essential Functions
- Develop and maintain a service program sufficiently flexible and dynamic to meet the needs of the under-served and economically disadvantaged populations, particularly those impacted by behavioral health and/or food insecurity.
- Develops partnerships and collaborations to plan and execute community prevention actions.
- Facilitates communication among the health center leadership, staff and the community.
- Participates in meetings, webinars, and calls as required.
- Maintains policies, principles, and procedures that agree with program requirements, guidelines, regulations, and directives.
- Implements weekly/monthly and/or quarterly tracking system to ensure budget, program, and compliance goals are met or adjusted as needed.
- Train the team to meet program requirements and directly supervises assigned staff.
- Direct, supervise and train staff on mental health and substance abuse activities, food insecurity activities, and leading team and service planning meetings, providing direction to staff regarding individual cases, conducting side-by-side contacts with staff, conducting individual supervision meetings and evaluating employee performance.
- Bilingual skills (English/Spanish) required
Current Texas certification as a Community Health Worker
- Must have at least two years paid supervisory experience
- Basic knowledge of a public health and community perspective and approaches, emphasizing population health and community prevention strategies.
- Knowledge of health issues and related services, community resources, and working with other communities and/or populations as described within the program.
- Thorough knowledge of program development, implementation methodology, and research methods and techniques
- Experience with developing and implementing work plans in collaboration with teams and partners.
- Understands local culture, context, and politics.
- Experience working in a community setting serving an underserved community population.
- Highly organized, self-motivated, and knowledgeable of targeted communities and possesses excellent engagement skills.
- Ability to foster and effectively partner in collaborative community efforts.
- Ability to effectively communicate with service staff and providers, decision-makers, and community members, including translating complex topics into understandable messaging.
- Budget oversight and financial reporting.
- Ability to work independently and in groups.
- Skilled in public speaking and developing presentations.
- Working knowledge of MS Office and EPIC electronic health records.
