Pomelo Care is a multi-disciplinary team of clinicians, engineers and problem solvers who are passionate about improving care for moms and babies. We are transforming outcomes for pregnant people and babies with evidence-based pregnancy and newborn care at scale. Our technology-driven care platform enables us to engage patients early, conduct individualized risk assessments for poor pregnancy outcomes, and deliver coordinated, personalized virtual care throughout pregnancy, NICU stays, and the first postpartum year. We measure ourselves by reductions in preterm births, NICU admissions, c-sections and maternal mortality; we improve outcomes and reduce healthcare spend.
Role Description
Your north star: Lead Pomelo's program to support NICU families across the care continuum, strengthening families' ability to care for their infant and reduce avoidable NICU days, readmissions, and costs.
In this role, you will build on and operate Pomelo's NICU Family Engagement program by providing direct family support, collaborating with internal and external stakeholders, and effectively closing the loop and tracking care for families. Specifically, you will:
Serve as a central point of contact for NICU families throughout the care continuum
  • Support families during NICU stays, leveraging evidence-based Family Engagement programs to support and empower families to care for infants during and after their NICU stay
  • Lead patients through a NICU discharge checklist to prepare for a timely discharge
  • Support families with transitions home, including working with hospital discharge coordinator and patient leading up to and in the days of transition
  • Follow up after discharge to connect families with resources and support, ensuring their ability to stay home and reduce avoidable readmissions

Leverage strong project management and cross-functional collaboration skills to coordinate with internal and external teams, ensuring NICU families are well-supported from admission to discharge
  • Collaborate with other Pomelo clinicians, including neonatologists, lactation consultants, perinatal mental health specialists, to ensure that evidence-based protocols are utilized in treatment plan
  • Use various data sources (HIE, patient reports, care team collaboration, claims) to identify and track patients in the NICU
  • Collaborate with health plan Case Managers and/or hospital discharge coordinators as appropriate to minimize duplication and ensure communication

Who you are
  • 5+ years progressive work experience in a Level IV Neonatal Intensive Care Unit
  • 5+ years of progressive work experience as a NICU Case Manager
  • Proven track record of connecting with and engaging patients with empathy in difficult times
  • Passion for working directly with patients and families and deep experience working with vulnerable populations (e.g., effectively engage multilingual families through language line)
  • High accountability and strong project management skills, with proven track record of tracking and owning follow-up across multiple teams
  • Extremely organized with meticulous attention to detail
  • Ability to track and clean data in Excel and use formulas/pivot tables to aggregate data results
  • Strong cross-functional partner; pleasantly persistent, with demonstrated ability to lead without authority across functions and teams

Bonus points if you have the following
  • Bachelor's degree in healthcare administration, nursing, or a related field. Master's degree preferred
  • Prior experience building national-scale clinical programs
  • Certification in case management

Our salary ranges are based on paying competitively for our company's size and industry, and are one part of the total compensation package that also includes equity, benefits, and other opportunities at Pomelo Care. In accordance with New York City, Colorado, California, and other applicable laws, Pomelo Care is required to provide a reasonable estimate of the compensation range for this role. Individual pay decisions are ultimately based on a number of factors, including qualifications for the role, experience level, skillset, geography, and balancing internal equity. A reasonable estimate of the current salary range is $85,000-$100,000. We expect most candidates to fall in the middle of the range. We also believe that your personal needs and preferences should be taken into consideration, so we allow some choice between equity and cash.
