ESSENTIAL FUNCTIONS:
- Admission Assessment: Conduct comprehensive assessments of new hospice patients, including physical, emotional, and psychosocial needs. Collaborate with the interdisciplinary team to determine eligibility and appropriateness for hospice care.
- Admission Coordination: Facilitate the admission process, ensuring all required documentation is completed accurately and timely. Collaborate with physicians, case managers, and families to establish a personalized care plan.
- Education and Support: Provide education and emotional support to patients and families regarding hospice services, expectations, and end-of-life care options.
- Resource Support: Act as a resource for the clinical team, offering guidance on symptom management, pain control, and other aspects of hospice care. Assist in coordinating resources, such as durable medical equipment and medication orders. When not admitted, assist with Crisis Intervention, Medication Management, Wound Care, Symptom Management, and PRN visits.
- Documentation: Maintain accurate and detailed patient records in compliance with regulatory standards. Ensure timely and appropriate documentation of patient assessments, care plans, and interventions.
- Continuous Improvement: Participate in quality improvement initiatives to enhance the quality of care provided. Stay updated on hospice care best practices and share knowledge with the team.
- Collaboration: Collaborate with other healthcare professionals, including physicians, nurse practitioners, social workers, and chaplains, to provide comprehensive care to patients.
QUALIFICATIONS:
- Registered Nurse (RN) license in the state of Texas.
- Minimum of 2 years of nursing experience, with hospice or palliative care experience preferred.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Compassionate and empathetic approach to end-of-life care.
- Knowledge of hospice regulations and guidelines.
- Ability to work independently and as part of a multidisciplinary team.
- Proficient computer skills for documentation and communication.
ENVIRONMENTAL AND WORKING CONDITION:
- Works in patients home in various conditions; possible exposure to blood and body fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather.
