Position Summary:
The Field Licensed Vocational Nurse (LVN) is a key member of our Home Health Agency team, responsible for delivering compassionate and skilled nursing care to patients in their homes. The primary duty of the Field LVN is to visit patients at home and provide necessary services in accordance with the established plan of care, ensuring optimal patient outcomes and satisfaction.
Responsibilities:
Patient Care Delivery:
· Visit assigned patients at their homes and deliver skilled nursing services as outlined in the individualized plan of care.
· Administer medications, treatments, and procedures as prescribed by the physician and within the scope of LVN practice.
· Provide wound care, catheter care, and other specialized treatments as required.
· Assist patients with activities of daily living (ADLs) as needed, including bathing, dressing, and mobility assistance.
Assessment and Monitoring:
· Conduct comprehensive assessments of patients' health status during each visit, including vital signs, wound status, and overall well-being.
· Monitor changes in patients' conditions and promptly report any concerns to the supervising Registered Nurse (RN) or physician.
· Document assessment findings, interventions, and patient responses accurately and in a timely manner.
Collaboration and Communication:
· Collaborate effectively with the interdisciplinary team, including physicians, RNs, therapists, and social workers, to coordinate patient care and ensure continuity of services.
· Communicate relevant patient information, changes in condition, and care plans to team members during case conferences and team meetings.
· Engage in open and respectful communication with patients, families, and caregivers to address their concerns and promote adherence to the plan of care.
Patient Education:
· Educate patients and caregivers about their medical conditions, treatment plans, and self-care techniques.
· Provide instruction on medication administration, potential side effects, and strategies for managing symptoms.
· Promote health literacy and empower patients to participate in their own care decisions.
Documentation and Compliance:
· Maintain accurate and thorough documentation of nursing assessments, interventions, and patient responses in accordance with agency policies and regulatory standards.
· Ensure compliance with infection control practices, safety guidelines, and HIPAA regulations to safeguard patient confidentiality and promote a safe care environment.
· Adhere to agency policies and procedures regarding documentation, scheduling, and professional conduct.
Job Types: Full-time, Part-time
Pay: $35.00 - $45.00 per hour
Expected hours: 20 – 40 per week
