SalaryWe are looking for a Field LVN Nurse to join our team and provide home health services to our patients. The successful candidate will be responsible for providing nursing care to patients in their homes and coordinating with other healthcare professionals to ensure the best possible care. This position requires excellent communication skills, working independently, and a commitment to providing quality care.
Responsibilities:
- Provide acute care services to patients in their homes.
- Provide home health care services to patients.
- Assess the patient's condition and follow a plan of care.
- Administer medications and treatments as prescribed by the physician.
- Provide education to patients and families on disease management and self-care.
- Maintain accurate patient records.
- Collaborate with other healthcare professionals to coordinate care.
Job Types: Full-time, Part-time, Per diem
Pay: $35.00 - $36.00 per hour
