Description
Provide case management support to complex care and non-compliant patients. Provides community outreach to at risk populations, including homeless, uninsured and youth. Administers Emergency Room diversion efforts and Managed Care Organizations' patient assignments. Using clinical best practices and disease registries provides population health management. Additionally, as needed, under the direction of a Registered Nurse or licensed medical provider, perform clinical functions within the scope of practice, including, but not limited to immunizations, performance of vital signs, hearing and vision screening, and medication dispensing. In addition, the incumbent may travel via personal vehicle throughout the community as well as between other Clinical facilities and departments.
Requirements
Education:
- Graduate of a Licensed Vocational Nursing Program.
License/ Certifications
- Valid California Licensed Vocational Nurse license.
- Provider level basic life support (BLS) from AHA approved program.
Experience:
- One-Two (1-2) years of LVN experience
Knowledge/Skills/Abilities:
- Demonstrate a commitment and passion for working with underserved patient populations.
- Demonstrates the ability to read, write and speak English and Spanish fluently.
