The LSU Healthcare Network is a non-profit, academic, multi-specialty, healthcare delivery system dedicated to patient care, research and education. Business Hours: Monday through Friday

INTRODUCTION

The Patient Navigator functions as part of the cardiology team as an advocate, interpreter, educator and counselor for patients. Their role is to coordinate patient care though the continuum in collaboration with other members of the team. Serves as a clinical resource with expertise in specialty care management. Serves as a liaison throughout associated facilities and the community regarding services provided for by LSU Cardiology. The successful Patient Navigator will be part of a team of healthcare professionals that is committed to delivering the highest quality of care to our patients. The primary role of the Patient Navigator will be to facilitate access of patients to the right providers across multiple clinic sites, [AJ1] facilitate scheduling of procedures at the various locations, field questions from patients regarding planned procedures and serve as a resource for concerns, queries, instructions and planned follow-ups.

JOB RESPONSIBILITIES

  • Coordinates written and verbal communication relating to the medical treatment plan between the patient, family, physician, staff and other team members. Communicates and coordinates the treatment plan to external and internal departments and agencies as needed to establish and maintain the continuity of patient care. Assists patients in understanding their diagnosis, treatment options, and the resources available. Serves as an essential link between patients and all other providers.
  • Facilitate appointments for consults and support services within established service standards, including social works, PT, OT, cardiac rehab, nutrition, financial resources, and referrals to other specialty services.
  • Follow patients throughout the care continuum, including inpatient admissions and collaborate with inpatient care management resources.
  • Work with service line personnel to collect data, track outcomes and support strategic planning processes.
  • Demonstrate the knowledge, skill and coordination to provide nursing care and guidance to the patient from screening to survivorship.
  • Assists patients and their family members “navigate” the system by helping them find solutions to treatment-related problems.
  • Uses the appropriate patient education materials regarding the disease process, diagnostic tests, planned therapies, potential side-effects and community resources.
  • Supports the patient during difficult decision-making periods.
  • May initiate and perform ongoing review of policies related to service provided. Where appropriate, updates or writes new policies to enhance professional practice.
  • Serves as a resource for community educational events such as health fairs, screenings, symposiums and lectures as well as staff education.
  • Maintain patient confidentiality
  • Participate in team meetings and training sessions
Requirements:

EDUCATION and/or EXPERIENCE

  • High School Diploma or GED
  • Successful completion of an accredited LPN or RN program.
  • 2 years of professional nursing experience in Cardiology is required.

CERTIFICATES, LICENSES, REGISTRATIONS

  • Current LA LPN or RN license required
  • Current CPR Certification required.
