Position Summary:
Under the leadership of the Director, Nursing, the Supervisor, Licensed Practical Nurse (LPN) is an active member of the department that collects data, provides comfort measures and basic care to patients. The Supervisor is responsible for ensuring the LPN staff provides care that is appropriate for the residents’ developmental age and is culturally sensitive. The Supervisor, LPN ensures the residents’ plan of care is followed and updated.
Basic Qualifications:
- Experience:
Requires one to two years of work-related experience or any equivalent combination of education, training and experience.
- Licensure, Registrations & Certifications:
Requires a current license as a Licensed Practical Nurse and is listed in good standing with the state’s Department of Professional Credentialing.
Requires and maintains a current CPR certification.
Essential Job Responsibilities:
- Contributes to the development of the nursing care plan through planning, data collection, appropriate implementation and evaluation of the resident care, as evidenced by resident, staff and supervisor observation.
- Supervises the delivery of quality resident care services by assigned staff that is consistent with the standards of care established by Nursing Services and CRHS.
- Functions as a part of an interdisciplinary team with positive interpersonal communication skills when communicating with residents, families, visitors, physicians and intra and inter-department staff members and representatives from other organizations.
- Ensures the Documentation of resident care which reflects nursing intervention, resident response to care provided, and includes evidence of appropriate resident/family education and the understanding of these instructions noted in the medical record.
- Ensures the implementation and documentation of resident responses to treatments/medications as defined within the scope of the LPN duties through the State Board of Nursing and policies of CRHS.
- Makes decisions and sets priorities that enable the completion of typical resident assignments within an assigned shift.
- Assumes responsibility for safety, infection control and performance improvement as part of CRHS’s efforts to provide quality care and services, and performs all associated duties, responsibilities and tasks.
- Reviews documents and reports prepared for the medical staff detailing changes in a resident’s condition.
- Supervises the operation of medical equipment in a manner that ensures the residents’ and staffs’ safety.
- Oversees the distribution of all prescribed medications to assigned residents in accordance with medical staff orders.
- Oversees the control of narcotics, drugs and supplies used in the delivery of care, and reviews inventory counts of all controlled substances.
- Complies with CRHS and department safety, infection control and performance improvement initiatives to ensure the delivery of quality care and services.
- Presents a professional image to CRHS’s nursing co-workers and clinical and medical staff members, residents, families and visitors.
- Attends professional develop programs and in-service training and meets the minimum requirement for continuing professional education.
- Works as a floor nurse during staffing challenges.
- Performs other related job duties as assigned.
Standards of Performance:
- Oversees the Performance of nursing care in accordance to all established department and CRHS policies and procedures as measured by feedback and observation from nursing, medical and management staff.
- Ensures that all assigned resident care duties are performed in a timely and accurate manner as measured by logs, charts and observation.
- Complies with all safety and infection control policies and practices in the delivery of direct resident care as measured by feedback and observation from nursing, medical and management staff.
- Demonstrates reliability as measured by compliance with the CRHS Time and Attendance policy.
- Demonstrates professional conduct and adherence to all safety, confidentiality and HIPAA standards.
- Promotes teamwork among subordinate staff, as well as other contacts within the Nursing Home as measured by observations of manager and coworkers.
- Adheres to the CRHS Standards of Behavior as measured by observations of staff, as well as outside and other inside customers.
- Verbalizes all core competencies of the unit and department.
- Actively participates as a member of the healthcare team by attending all mandated in-services and a minimum of 75% of the unit staff meetings and employee forums.
