- Provide direct patient care in the patient's home and school
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and assess overall health status
- Perform routine procedures such as wound care, catheter care, and g-tube care
- Educate patients and their families on health promotion and disease prevention strategies
- Document patient information accurately and maintain confidentiality
```Qualifications:```
- Valid Licensed Practical Nurse (LPN) license in the state of [Location]
- Proven experience administering injections and performing other nursing procedures
- Strong knowledge of acute care practices and patient assessment techniques
- Excellent communication and interpersonal skills
