Title: LPN (Licensed Practical Nurse)

Function: To provide comprehensive medical support to each supported individual. Under the direction of the Director of Nursing, the LPN will work within her entire scope of practice and be fully accountable for ensuring that competent care is being consistently delivered to all supported individuals. LPN is considered the manager on shift when program manager is not available.
Health:Support our individuals to have optimal health and well-being. Provide bedside care, administer medications & treatments, document therapeutic effects, take vital signs, assist with individual comfort, change colostomies, tracheotomies, prepare enemas and catherization’s, collect samples, and monitor G/J tubes. Attend Physician appointments when needed. Coach, guide and manage DSP to provide optimal health. Ensure our individuals are free from abuse & neglect. Complete all necessary reporting and documentation.
Safety:Keep our individuals safe. Consult and follow RN care plans. Safely assist our individuals throughout their day. Supervise meal preparedness and mealtimes based on personal needs. Report unsafe conditions to the Program Manager. Follow all safety policies to foster a safe working environment.
Time:Positively represent our individuals’ skills to succeed. Uphold agency mission & values. Understand and follow through with agency policy and procedures. Communicate positively and clearly with Managers/co-workers, physicians, guardians, advocates, family members and others involved in the care of the people we support. Complete required training in timely manner. Maintain satisfactory attendance and complete all assigned tasks on shift. Maintains adequate medical inventory in the program.

Support our individuals with their daily living skills. Role-model positive behavioral techniques to promote dignity and respect.

Qualifications:
- Valid Practical Nurse License from State of CT
- Valid CT Driver's License
- Mandatory Flu Vaccinations
Job Types: Full-time, Part-time, Per diem
Pay: From $28.00 per hour
