Dosberg Manor

1. JOB SUMMARY:

To ensure the safety and well-being of the residents in Dosberg Manor, a 96 bed Assisted Living facility, through nursing assessment, quality assurance, and oversight of the Dosberg Manor staff.

1. ESSENTIAL DUTIES:

• Works closely with the governing body and management to ensure effective functioning of facility enforcing appropriate policies and procedures.

• Assists in the development and on-going process of Quality Assurance/Quality Improvement and completes audits as assigned.
• Assists in the review and update of all nursing policies and procedures.
• Assumes on-call responsibilities, works additional shifts in order to meet staffing requirements.
• Monitors and updates resident charts, monthly MAR’s, annual medical evaluations and plan of care as needed.  Monitors physician and pharmacy information.
• Makes periodic rounds to all residents to assure proper services are being provided on an on-going basis.
• Plans and provides for the over-all safety and well being of the residents.
• Serves as a resource to Unit Supervisors, HHA’s and PCA’s.  Provides counseling and discipline according to guidelines.  Prepares and presents evaluations for the Unit Supervisors and the care aides.
• Prepares the monthly schedules for the Unit Supervisors and coordinates proper staffing for the care aides.
• Participates in new employee orientation and ensures completion of new-hire paperwork.  Conducts regular medication training classes for care aides and ensures their qualifications for medication pass.
• Conducts necessary in-servicing and educational training for Dosberg Associates

1. OTHER JOB DUTIES:
   • Maintains Accident and Incident reports and log, submits to DOH as necessary.
   • Pre-screens potential applicants for admission to Dosberg Manor.
   • Attends team meetings to review Plan of Care.
   • Acts as a standing member of the Quality Assurance Committee.
   • Responsible for the coordination and destruction of discontinued narcotics per NYS Bureau of Controlled Substances guidelines.
   • Performs other duties as assigned.

1. WORKING CONDITIONS:
   • Potential for exposure to blood borne pathogens.
   • Has full and complete access to the resident’s medical record and reports where utmost integrity is required to safeguard resident which if disclosed would have adverse effects.

• • Standing, walking for short periods of time
  • Able to lift and carry 50 pounds
  • Mental/Visual Demand and Physical Effort:         
  • Flow of work frequently produces a high level of mental/physical fatigue. 
  • Moderate physical effort required. 
  • Requires intense mental application and documentation requires attention to detail.

1. EDUCATION:
   • Current Licensed Practical Nurse License to practice in NYS

1. EXPERIENCE:

• FIVE (5) years of experience as an LPN (required)
• TWO (2) years of experience in a supervisory position (preferred)
